Paris Skilled Nursing Facility provides the best in patient care

Chris Johnson operated Paris Skilled Nursing Facility (PSNF) from September, 2005 – October 3, 2011 as the Administrator. During which time we received deficiency-free Health Code and Life Safety Code Survey in 2006 and in 2008. Was award the highest level of ranking as a 5-Star facility by the Centers of Medicare and Medicaid Services. Additionally, PSNF became ranked in the top 10% by consumer reports as the “The Best in Texas”.

As of July of this year, Chris Johnson is back as the Administrator at Paris Skilled Nursing Facility and is looking forward to serving the patient and the people of our community.

OUR GOAL IS TO GET YOU BACK HOME!

We don’t want to be the best kept secret in town anymore, so we have compiled a list of circumstances that would allow you to be eligible to come to our unit.

            ELIGIBILITY

3 midnight hospital stays qualifies you for 100 days of benefits.

Must be in need of skilled nursing or rehab service.

If you go home from a Hospital or SNF and have a decline, you would be eligible to come to us within 30 days of discharge date and your hospital stay would be waived.

If you are currently in a skilled nursing facility and are not satisfied with the care being

provided, you would be eligible to transfer to our unit.

If you are coming home from Dallas, Tyler, or any other Hospital and you’re just not quite ready to be home, we will be more than happy to have you here at our unit for a short stay.

You would be eligible for a new 100 day benefit period if you have been free of any

hospital stay for 60 days.

            ADDITIONAL SERVICES

Part B outpatient therapy services are also available with Medical Doctor Prescription.

If you’re comfortable with your professional team of caregivers at our unit why change?  You don’t have to!   Upon your request, we will enhance the continuity of our specialized

services by  allowing our therapists and nurses to follow you at home through our home

care services division.

100 DAY BENEFIT PERIOD

Day 1-20 Medicare pays 100%  -  “$0 out-of-pocket”.

Day 21-100 Medicare pays all but $144.50 per day.

Most all Medicare supplements including Medicaid will cover the $144.50 per day.

Though individual plans vary, there is no need to worry because our business office will verify and inform you before any out-of-pocket expenses are incurred.
